D = Did the team or individual forget we are not supposed to do goal-orientated diving

I like the idea of doing debriefs, and we definitely do after every training dive, but I'm not sure it needs to be this formal. I think this might be a solution looking for a problem. However, I am always open to new ideas so I'll see what the chimps think of it.
