For info, if anyone does this either by fax or email, bear in mind the different US paper sizes if sending from Europe. A friend of mine faxed his outline straight to San Diego when he got his Sig Series, but it distorted a little printing out in "letter" rather than "A4" and they put the wrong size boots on his suit as a result.

It would be an idea therefore to draw lines on the outline, heel to toe and across the ball of the foot, measure these (use inches :D ) and write them on there, and draw their attention to the possibility of distortion.
